Practicing under the direction of your physician, our highly trained licensed nurses come to your home and thoroughly evaluate and establish an individualized plan of care that will address all medical and personal care needs. Our nurses will continue visiting you at your home until your goals are met.

Physical Therapy

If you have been recently hospitalized with an illness or injury, are experiencing weakness, or are at risk of falling, your physician may request a physical therapy evalutaion. 

 

A licensed physical therapist will evaluate you at your home, establishing an indivualized care plan, setting goals, making safety recommendations, and implementing daily exercise routines, in order to restore balance and improve mobility.

Occupational Therapy

Occupational therapists evaluate the patient’s ability to perform activities of daily living and develop programs that help restore function and improve fine motor skills.

 

Patients who have had a stroke or cardiac episode, spinal cord injury, arthritis or low vision may benefit from occupational therapy services.

                        

Speech Therapy

Illnesses such as stroke can lead to challenges with communcation or swallowing. Our speech therapists understand and want to help you overcome these challenges, by enhancing your strengths and minimizing self-doubt. We begin therapy with self-belief.

 

Medical Social Work

Our Medical Social Worker is here to assist with short or long term planning for you or your loved one. We will begin by assessing your needs and then making referrals to appropriate community resources. If you have questions, we have answers.

Home Health Aides

Our highly qualified Home Health Aides are typically sent in conjunction with a therapy service. They aide the patient with activities of daily living such as bathing, dressing and grooming.
